Request Assistance
The Sanneh Foundation accomplishes our distribution strategy by networking with established non-profits, soccer clubs and other related organizations. In order to ensure that each donation is used properly, we will review each Donation Request we receive and will approve based upon the stated Purpose and Mission Statement of the organization, as well as an understanding of how the donation will make a difference.
To be able to successfully fill a request, you must complete and submit the Distribution Request Form below, which provides us with the pertinent information to fulfill your request to the best of our ability.
Steps to follow…
- Read the Distribution Agreement.
- After reading the Distribution Agreement, complete and sign the Distribution Request Form. Please print clearly when completing the form. Sign and mail the form to the address at the bottom, or fax the form to 612-605-1934.
- This form will be kept on file and reviewed once your organization/request is approved. Once approved please allow up to 30 days until the footwear is shipped from our primary warehouse in Minnesota. You will be notified upon approval, of the time frame for fulfillment of your request. We always want to provide footwear that meets your needs, so this timeframe will vary with availability.
